Running two vials at once is sometimes unavoidable: two compounds, or one compound at two concentrations during a transition. It is also the situation in which the arithmetic most often produces a real error, because the two vials look identical and the syringe cannot tell them apart.

Here is the shape of the hazard. Vial one is 5 mg in 2 mL, so 2.5 mg/mL. Vial two is 10 mg in 2 mL, so 5 mg/mL. Your dose from vial one is 0.25 mg, which is 0.1 mL, which is 10 units. Ten units drawn from vial two is 0.1 mL times 5, which is 0.5 mg. The syringe looks exactly the same, the liquid looks exactly the same, and the delivered dose is double.

The mitigation that actually works is to make the two vials produce different unit counts, so that a wrong draw looks wrong. If you reconstitute vial two into 4 mL instead, both vials are at 2.5 mg/mL and 10 units is 0.25 mg from either, which removes the hazard entirely at the cost of a larger volume in the second vial. If the concentrations must differ, then arrange the doses so that one is drawn at, say, 10 units and the other at 30 units; a habitual reach for the wrong number is then visibly off rather than plausible.

Label both vials with concentration in mg/mL and the dose in units, on the vial body rather than the cap, since caps get swapped. Writing the mix date matters too, because with two vials in rotation the older one will be finished later than you expect and you will want to know how long it has been open.

Physical separation is worth more than any labelling scheme. Different shelves, different boxes, different coloured tape. The error mode is not misreading a label, it is not reading it at all because your hands already know where the vial lives.

Before the first draw from a second concurrent vial, work its full ladder once: at 5 mg/mL, 0.25 mg is 5 units, 0.5 mg is 10 units, 0.75 mg is 15 units, 1 mg is 20 units. Having those written down where you inject means you are reading a number rather than deriving one at the point of use.
